Stephanie, generally, I have patients take 20 to 150mg a day for sleep +/- anxiety. Start low and go slow. Know the dosages of your product. Usually 2/3 to 3/4 of the daily dose is 1-2 hours before bedtime, and the other portion is upon waking (to improve wakefulness during the day). Other factors such as stress, hormone replacement, other meds & medical conditions, etc. play a role along with individual differences. I own a compounding pharmacy, so we see a lot of unique needs. I can't give more specific advice in this forum, but there is help!

The discussion on the legal status of CBD revolves mainly around the question: is it a medicine or a natural food supplement? The main difference is that medicinal drugs are considered unsafe until proven safe, whereas food supplements are considered safe until proven otherwise. As a result, the central question becomes whether or not CBD is safe for consumers (children, elderly, patients) in large and unregulated quantities. Although there is only limited knowledge about the long-term effects of chronic use, or about drug-drug interactions between CBD and other medications [36], human studies have indicated that CBD is very well tolerated even up to a daily dose of 1,500 mg [37]. An excellent example is the use of CBD (and also THC) products for the self-medicating of cancer, with the intention of fully curing it [15]. This is based on an increasing body of preclinical evidence showing cannabinoids to be capable, under some conditions, of inhibiting the development of cancer cells in vitro or in vivo by various mechanisms of action, including induction of apoptosis, inhibition of angiogenesis, and arresting the cell cycle [16]. This is certainly exciting news, and research is ongoing around the world, but there is no solid clinical evidence yet to support that cannabinoids – whether natural or synthetic – can effectively and safely treat cancer in actual humans [17]. In fact, there are indications that certain types of cancer may even accelerate when exposed to cannabinoids [18]. This becomes problematic when patients choose to refuse chemotherapy treatment because they firmly believe in the rumored curative properties of cannabinoids. As a result, recommendation of cannabinoids for treating cancer should be done with great care, and with distinction as to the type of cancer being treated [19].

Scientifically, the difference between what we refer to as hemp and marijuana comes from the intention the strain was bred for. The main difference between hemp and marijuana is the level of THC in the plant. Hemp has high levels of cannabidiol (CBD) and very low levels of tetrahydrocannabinol (THC). Marijuana is recognized for having high levels of THC and lower in CBD. In order to be considered hemp, the plant must have less than 0.3 percent THC. I suffer with migraines, fibromyalgia, and arthritis. I pretty much hurt anytime I move. I’m on a regimen of meds, but ordered the Virgin Cannibis Hemp Oil (off of Amazon) and have had it two days. I took 3 tablespoons a day, gagging it down. I couldn’t bring myself to take it today, but I didn’t notice a difference after the second day, and I suppose I would notice something. Was it too soon, or should I be trying a different product? I really need something to alleviate the level of pain during movement, as it’s causing me to be more sedentary.

The amount of CBD needed to produce significant effects varies by condition, according to both Blessing and Carson. “In clinical trials for schizophrenia they would give 800 milligrams,” says Blessing. “In our clinical trial to treat PTSD, we are giving 600 mg per day.” In the clinical trial for Epidiolex, Carson says, the clinicians administered 10 milligrams per kilogram the subject weighed — meaning a person who weighs 50 kilograms (or 110 pounds) would be given 500 milligrams of CBD.

Topical CBD can also work faster, since it doesn’t have to travel through the digestive system first.
Two cannabis-based pharmaceutical drugs, manufactured in the UK, are licensed for prescription but only for very specific uses. Sativex has been available in the UK since 2010 and uses THC and CBD to treat spasticity in multiple sclerosis. And a new CBD-only drug, Epidiolex, was approved in June in the US to treat rare childhood epilepsies, with a similar decision expected imminently for Europe and the UK.

Ingesting — think CBD lattes, edibles, or just a drop of oil on the tongue — is likely much less effective than inhaling, says Blessing. When CBD-containing oil is ingested, it wants to join the other fat in your body; most of the CBD taken this way will just stay in that fat, inert and never getting to the brain. When inhaled, CBD bypasses the digestive system, which wants to store fat.
Like many of my fellow New York City residents, I’ve recently grown aware of the presence of CBD in my surroundings. In particular, CBD at coffee shops. It started when some friends of mine were discussing a pilgrimage out to Flower Power Coffee Co., known for its “artisanal CBD-infused coffee.” Then, at a fancy matcha cart I went to for Instagram-related purposes, I noticed that alongside health food add-ins like oat milk was the option to include a one-dollar shot of CBD. I didn’t do it, because weed in most forms tends to make me sleepy and it was 1 p.m. on a Saturday in the middle of Brooklyn's trendiest neighborhood.
Either way, it would be hard to script a more of-the-moment salve for a nation on edge. With its proponents claiming that CBD treats ailments as diverse as inflammation, pain, acne, anxiety, insomnia, depression, post-traumatic stress and even cancer, it’s easy to wonder if this all natural, non-psychotropic and widely available cousin of marijuana represents a cure for the 21st century itself.
Unlike with THC, CBD’s effects aren’t limited to that single receptor. These effects are not precisely known, though CBD certainly has some impact on CB1’s sister receptor (CB2) as well as a receptor called 5-HT1A. When the 5-HT1A receptor comes into contact with a material that agonizes it, the effects can include reduced anxiety and increased calmness.
Further explanation is in order. The difference between psychoactive cannabis (“marijuana,” by the increasingly disfavored traditional term) and hemp is a question of THC content — and legal classification. The federal government defines hemp as cannabis with 0.3 percent THC or less in terms of dry weight. The 2014 federal Farm Bill legalized state pilot programs for hemp, and 40 of the 50 states now have defined industrial hemp as distinct from cannabis and removed barriers to its production — including California. However, California’s hemp pilot program is overseen by the California Department of Food and Agriculture (CDFA) — not either the CDPH or Bureau of Cannabis Control.
In September 2018, following its approval by the FDA for rare types of childhood epilepsy,[13] Epidiolex was rescheduled (by the Drug Enforcement Administration) as a Schedule V drug to allow for its prescription use.[14] This change applies only to FDA-approved products containing no more than 0.1 percent THC.[14] This allows GW Pharmaceuticals to sell Epidiolex, but it does not apply broadly and all other CBD-containing products remain Schedule I drugs.[14] Epidiolex still requires rescheduling in some states before it can be prescribed in those states.[66][67]
Cannabidiol is currently a class B1 controlled drug in New Zealand under the Misuse of Drugs Act. It is also a prescription medicine under the Medicines Act. In 2017 the rules were changed so that anyone wanting to use it could go to the Health Ministry for approval. Prior to this, the only way to obtain a prescription was to seek the personal approval of the Minister of Health.
